I think the Quasar "mistake" in the Infinity Gauntlet is he's drawn throughout in his old costume when his look was updated in his own book right when Infinity Gauntlet started. A subsequent issue of Quasar specifically makes an allusion to the appearance in Infinity Gauntlet by having Wendell showing up wearing that old costume and upon realizing his mistake says he keeps forgetting about his new costume and even accidentally wore his old costume "for a whole day" earlier in the week.
