Do characters get killed in order to get reaction from other characters (read: male angst) or is reaction of other characters natural consequence of death and character is killed because of killing is fun or to get a reaction from reader?

Characters are killed as a function of whatever story they’re killed in–because the creators and editors working on that story feel that the death is justified and a natural outgrowth of what is going on. There are a million different shades of how that breaks down–the real answer from your list is “all of the above to some degree.” But the percentages of how much one as opposed to another may be quite different.
