Given the sheer amount of unique multiversal characters and visitors from other realities such as Captain Britain, Kang, AoA inmigrants, Rachel Summers, Bishop, the incursions from New Avengers and countless others, isn't earth 616 completely unique and thus all alternate realities are entirely different, not just divergent?

No, not necessarily. Because we don’t get to see any of those other realities in the same degree of detail as the regular Marvel Universe, it’s impossible to compare–they may have just as many or more multiversal characters and visitors and so forth. What they don’t have is a line of comics about them.
