An assumption the writer knows best, a prioritising of scheduling over smaller details that the majority of the audience may not be distracted by or just "mistakes happen, do better next time".

    A little from column A, a little from column B, a little from column C. But almost all mistakes happened because nobody in the chain of creation was aware of something, rather than because a specific choice was made to get something wrong.
