My understanding of the ending of Inferno is that Jean, Madeline and the Phoenix Force all became one. 1. Why was John Ostrander allowed by editorial to resurrect Madeline Pryor when that needlessly complicated things and 2. Why was having X-Man date his own mother allowed past editorial? 3. Is this at least part of the reason why Old Jean hasn't returned?
1) Because he was telling a story.

3) Not at all. Jean was alive when John did that story. And now she isn’t.
