When a lettering credit reads "RS & Comicraft's [name of letterer]," does that mean the Comicraft staff letterer is the sole letterer, or that he co-lettered it with Richard Starkings?

Typically, it means that the letterer in question lettered the book using fonts developed by Richard and under his supervision.
